Payment for follow-up blood test??

Hello,
I remember when I submitted for the program, I paid $35 to get my blood drawn, in which I paid online directly on the DLS website.

How do I pay again to have the follow-up bloodwork? I received the requisition, but I am assuming I need to pay another $35??

Good morning JennaB,

Yes - you pay the same as before. When you logon to order your blood test they ask for your payment information then send you the requisition. Are you sure you've received the requistion and not just the form to request a requisition? Perhaps they errored and processed without charging you? Very interesting... I never received a requistion without being charged first.

OK, so I realized that I don't have the requisition, but just the letter from Dr. Cohen. So, do I go directly to the DLS website??

I believe so. Follow the link that is included in the referal letter from Dr. Cohen and they'll ask you for your payment/credit card information. Good luck!

I don't see a link in the letter. It only states Dr. Cohen's email address.
I'm so confused.

The link for the blood test is accessed from your 1PD Personal page. From there, you pay the $35 and find a nearby lab.

I think I am definitely missing something! On my personal page, it has:
Personal Program
Your Customer Guide
Your Recipe Book
Blood Test 1
Bloodwork Referral Letter from Dr. Cohen

In this letter, it is addressed to "To Whom It May Concern", asking them to fax my results to his fax #.

I don't see where I pay the $35 directly to the lab, and can't remember for the life of me how I did the process when I registered.

Any help would be GREATLY appreciated!

In the upper right hand corner of your Personal page, under "Your Links" is a link to click that says "Blood Test Order." Follow that to place the order and make payment.

Hi,
I am afraid I did not!! :sadeyes:
I already had the referral letter from a couple of days ago. That was sitting in My Library. Just a few minutes ago, I clicked on "Blood Test Order" again to see if I could see the link. Instead, it is in the process of generating a new referral letter!

I am still unable to locate the link so that I can have the requisition generated.

I wanted to try to get to the lab tomorrow since Friday is a holiday in the US. But, I am afraid that I won't get the requisition from DLS in time!!

Any tips/suggestions??

This is the link that you get to when you follow my earlier instructions and click on the link for Blood Test - not the link for the Blood Test Referral letter. There are two links on the page and the top one is the correct one.

In the upper right hand corner of your Personal page, under "Your Links" is a link to click that says "Blood Test Order." Click this and you will be taken to a window where there are two choices of links to click. The TOP link takes you to the requisition form and payment site. The BOTTOM link simply requests another referral letter.

Hi Jenna,

I didn't even know about the link either. I just went to the same lab as last time and brought the referrel letter. This time my blood test cost 75$ OUCH. I wasn't sure why there was the price difference but now I know.
